The Fraxel laser device allows you to reduce the severity of wrinkles, reduce the depth of scars after acne, eliminates age spots, and also treats many other skin problems. The Fraxel Cosmetology Laser is a fractional type laser and is available in three versions: "Fraxel re'fine", "Fraxel re'store" and "Fraxel re'pair".
The feature of fractional lasers is that they divide the laser beam into thousands of microscopic fractions (small rays, each of which has a size of one-tenth the diameter of the hair) and direct these fractions simultaneously to a specific area of skin. As you will see below, this skin treatment technology has global advantages.
Fractional laser can ablative and non-ablative. Fraxel restore and Fraxel refine - are non-ablative. These lasers work in the depth of the skin, without compromising the integrity of the stratum corneum of the epidermis. But "Fraxel repair" is ablative, and its impact occurs in violation of the integrity of the stratum corneum of the epidermis. The latter seems to be less secure, however, the efficiency of Fraxel repair is much higher than other Fraxel machines.
Fraxel: indications for the use of a fractional laser
The Fraxel, like many other fractional lasers (manufactured under other trademarks), are commonly used to treat the following problems:
- to reduce wrinkles and deep lines on the face and neck (Fig.4-5),
- for general rejuvenation of the skin and increase its elasticity,
- stretch marks (striae) after pregnancy (Fig.6),
- scars and scars after surgery (Fig.7),
- acne scars (fig.8)
- enlarged pores
- age spots, melasma (Fig.9),
- spider veins,
- skin keratosis treatment.

Three modifications of the Fraksel laser -
There are three main original Fraxel devices that are used in different clinical situations, and depending on the severity of the problem.
1. Fraxel Re'fine -
This fractional non-ablative erbium laser, with a wavelength of 1410 nm and a single pulse energy from 5 to 20 mJ (Fig. 10). This laser operates at a depth of 0.7 mm, and the stratum corneum of the epidermis remains intact. This is the softest type of Fraksel laser, after which the fastest healing takes place, and the procedure itself is as painless as possible and lasts only 20 minutes.
Fraxel refine is used to smooth out fine wrinkles around the eyes, tighten the eyelid skin, remove age spots and other pigmentation on the skin, and treat melasma. Also, this type of laser generally improves skin condition and can be used primarily for the prevention of aging.
Thus, the indications for using Fraxel refine are the same as for IPL photorejuvenation techniques. The latter, as a rule, are much cheaper than the Fraxel laser treatment. However, Fraxel can be used in people with any skin color, and IPL devices are not recommended for people with dark skin.
2. Fraxel Re'store -
This fractional non-ablative erbium laser, with a wavelength of 1550 nm and a single pulse energy from 4 to 70 mJ (Fig. 11). The depth of exposure depending on the settings ranges from 0.4 to 1.4 mm. This type of laser is used to treat similar problems, which allows you to eliminate Fraxel refine, but only more pronounced. For example, to smooth more pronounced wrinkles.
Also, Fraxel restore reviews claim that it copes with the removal of small atrophic scars after acne, hypertrophic scars, and is also used to treat stretch marks after pregnancy, pigmentation and keratosis of the skin.
3. Fraxel Re'pair -
This is a fractional ablative laser, belongs to the group of CO2 (carbon dioxide) lasers with a wavelength of 10,600 nm and an energy of one pulse up to 225 mJ (Fig. 12). Unlike classic ablative lasers that work only on the depth of the epidermis - Fraxel Repair works in both the epidermis and the dermis. The depth of exposure is from 0.4 to 1.6 mm, and it can be adjusted by the settings of the device, which can be selected modes: "average ablation" or "deep ablation."
In the mode of medium ablation, the effect is mainly on the lower layers of the epidermis and the upper layers of the dermis, which is suitable for smoothing shallow wrinkles (for example, around the eyes), for reducing the pore size and improving skin texture.
Fraxel repair in the "deep ablation" mode affects mainly the middle layers of the dermis, which is great for smoothing deep wrinkles. When using this mode, there is also a pronounced skin lifting effect, which you will notice immediately after the procedure. Also, the device is successfully used to remove pigment spots and spider veins, as well as to remove hypertrophic scars and scars.
Analogues of the Fraxel Repair laser, produced by other manufacturers -
- Active and Deep FX (Lumenis),
- Quadralase (Candela),
- Pearl Fractional (Cutera), etc ...
Comparing Fraxel refine and Fraxel restore - with Fraxel repair ...
The Fraxel repair fractional ablative CO2 laser is more aggressive than the Fraxel refine and Fraxel restore erbium nonablative lasers, which leads to a greater synthesis in tissues of inflammatory mediators, proteins and factors that stimulate stem cells and the function of fibroblasts. Accordingly, the effectiveness of the procedure in an ablative CO2 Fraksel laser will be 2 times higher, although healing will take 2–3 times longer. In addition, it is "Fraxel repair" that gives the best skin lifting effect.

How Fraxel Fractional Laser Works -
To understand how all this works, it is necessary to know the structure of the skin (Fig. 13). The skin consists of 3 layers:
→ epidermis (surface layer),
→ dermis (middle layer),
→ subcutaneous fatty tissue.
The epidermis consists of numerous layers of keratinocytes, different degrees of differentiation. Also, the epidermis contains melanocytes (the so-called pigment cells), which are responsible for skin color.
The dermis consists of collagen and elastin fibers, fibroblasts, which provide the skin with strength, elasticity and elasticity (24stoma.ru).
With the aging of our body - the appearance and general characteristics of the skin change. The epidermis becomes thinner, the various spots on the skin become more visible, the collagen content in the dermis to 45 years decreases almost two. Visibly reduced skin moisture due to a decrease in the content of hyaluronic acid. All this leads to a gradual appearance of wrinkles, the formation of deep lines on the face, the skin becomes flabby.
Principle of operation -
The laser beam is divided into thousands of microbeams using a special technology, each of which is one-tenth the size of a human hair. That is why such lasers are called fractional. The meaning of such factionalism is that the areas of tissue being destroyed are subsequently surrounded by areas of intact, viable tissues, the cells of which will ensure the quick recovery of damaged areas.
When using non-ablative Fraxel lasers (Fraxel refine and Fraxel restore), numerous micro-sections of tissue coagulation (microthermal zones MTZ) are formed - in the form of columns perpendicular to the skin surface (Fig.14). And with the use of the Fraxel repair ablative laser, plus not only trauma occurs, but also a certain percentage of the tissue is removed, which leads to a pronounced skin lifting effect.
In response to such damage, the body begins to synthesize a larger number of proteins, mediators and signaling molecules, which cause activation of skin stem cells and increase the speed of their division. A large number of new cells begin to gradually synthesize the structural elements of the skin, such as collagen and elastin. And after the application of the Fraxel repair ablative laser, these processes are more pronounced.
When using non-ablative Fraxel lasers (Fraxel refine and Fraxel restore), coagulation sites occur predominantly in the dermis without compromising the integrity of the stratum corneum of the epidermis. And when using the Fraxel repair ablative laser, coagulation occurs already in violation of the integrity of the stratum corneum, which leads to the formation of microholes in it up to 200 microns in size, fully restored in about 24-45 hours (Figure 17). Damage to the dermis is restored in 4-7 days.
The steps of the procedure using Fraxel -
1. Preliminary training
The problem area is determined and adapted to the template to determine the target correction area. Be sure to make fraksel photo before the procedure. The patient should remove all jewelry and wash face thoroughly with soap and water before the procedure.
For patients with skin pigmentation problems, a whitening cream treatment is required (for example, hydroquinone) or prior chemical peeling may be required. In patients with a high risk of developing herpes, Acyclovir can be used as a prophylactic against herpes.
Next on the treated area is applied anesthetic. It is necessary to wait 45-60 minutes before the full anesthetic effect. Then the anesthetic is removed and a special gel is applied to the treatment area, which ensures that the Fraxel laser grip slides easily on the skin surface.
2. Fractional Laser Treatment Procedure
The treatment time depends on the area of treatment. For example, the full face usually takes about 30 minutes, the neck about 20 minutes. The degree of pain during the procedure will depend on the power of the laser light pulse energy (from 4 to 70 mJ; it is set on the equipment). Sometimes a cooling device is used to reduce discomfort during the procedure (for example, Zimmer).
Typically, 4-5 fraxel treatments are required, depending on the level of energy used and the condition of the treated skin. These procedures are performed within a month.
3. Fraksel: reviews and consequences
For about 1 hour after the procedure, patients may experience a feeling like after tanning in the sun. During healing and for several months after the procedure, a moisturizing sunscreen with a protection level of at least 50 SPF is recommended. It is necessary to protect the skin from sun exposure and with the help of clothing and wide-brimmed hats.
There may be edema, which usually disappears within 2-3 days. Get ready for skin to have a pink shade for 3-5 days. In addition, within up to 40 hours, the epidermis is restored, which at the beginning may have a tan color (this takes 3-14 days). As a rule, peeling of the skin is observed, but this problem is easily solved by applying a moisturizer. According to the indications of a cosmetologist after laser treatment with Fraxel repair, peeling of the skin with water, vaseline and gauze can be used 1-2 times a day.

Fractional laser: reviews
Results after Fraxel - reviews confirm that the onset of effect is progressive. The most visible result occurs within 3-4 months after the procedure. During this period, complete healing and replacement of tissues occurs with the formation of new cells with new collagen and elastin. However, after applying Fraxel repair - the lifting effect can be seen immediately after the procedure.
Studies show that after the procedure of easy rejuvenation, 20-40% of the overall improvement of fine wrinkles and lines was achieved. More aggressive procedures (with high energy of the light beam), which are performed by the fractional laser - reviews showed from 30 to 70% of the visible improvement.
Fractional laser can be combined with surgical methods of rejuvenation, as well as non-invasive procedures, such as plasma lifting. In the latter case, the desired result can be achieved faster and with fewer procedures. Laser skin resurfacing is not a cheap pleasure, so the number of treatments can also matter.
Side effects and complications -
Laser Fraksel reviews confirm that the procedure is usually fairly well tolerated by patients. In the near future after the procedure it is possible to shave and apply make-up. In most cases, patients can return to work immediately after treatment or the next day, which will depend on the laser pulse power chosen by the beautician.
Side Effects and Effects after Fraxel -
- excessive peeling of the skin (sometimes the appearance of crusts),
- contact dermatitis,
- acute herpes, acne,
- infectious skin lesions,
- temporary post-inflammatory skin pigmentation,
- swelling within 1 week after treatment (in such cases, the use of ice and antihistamines helps).
Important: Despite the fact that fractional laser is most often used to treat various pigmentation disorders, the treatment itself may in some cases lead to post-inflammatory pigmentation.
Fractional laser: price
Fraksel price for procedures in Moscow differs depending on the treatment area and the type of fractional laser itself. Below we indicated the cost of the procedure on the Fraxel restore device. Depending on the severity of skin changes, 3-5 procedures may be necessary to achieve the desired result.
Fractional laser: the price of the procedure
- the whole face, including eyelids, will average 25,000 - 30,000 rubles,
- only crow's feet near the eyes - about 4,500–6,000 rubles,
- nasolabial triangle - about 6000 - 8000 rubles.
